To provide you with an upgraded experience for requesting help from the practice and taking control of your health, Dr.iQ is now powered by Evergreen.
This means you can now access your GP services and request help from the practice via the Evergreen Life app.
What is Dr.iQ?

The new app brings all your health information, GP records and services into one place.
With the Evergreen Life app, you can:
- Order repeat prescriptions easily
- Access your GP records and results
- Request help from your GP surgery
- Get insights on how to be as happy and healthy as possible

Please make sure that you only use the links or QR codes on this page to download the Evergreen Life app if you are registered at The Potteries Surgery.
How do I access the app?
- Click here to download the app on your mobile device or scan the QR code below using your smartphone:

- Create an Evergreen Life account
- Link to your GP securely with NHS login to access your GP services
- Tap ‘Request help from GP’ to get support from a clinician
If you’re unable to download the app or don’t want to use online services, please contact the practice directly by phone to make a request.

Translation service
If you would like a translator to be present at your appointment, please phone the surgery.
Cancelling or changing an appointment
To cancel your appointment:
- use your NHS account (through the NHS website or NHS App)
- use the GP online system: Patient Access
- phone us on 0118 942 7528 during opening times
If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
If you need help with your appointment
Please tell us:
- if there’s a specific doctor, nurse or other health professional you would prefer to respond
- if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
- if you need an interpreter
- if you have any other access or communication needs
Home visits
If you are too unwell or infirm to visit the surgery, please telephone before 10am as this allows the doctors to plan their visits.
The receptionist answering your call will need to ask you certain questions regarding the complaint. These details are necessary to enable us to deal with the most urgent calls first.
Where possible you will be asked to attend the surgery, as facilities for examination and treatment are much better there.
