Learning Disabilities Register
It is important that we know if you, a member of your family, or someone you care for has a learning disability, so that we can make sure you receive information on the support available to you.
As well as providing a free annual healthcare check, the surgery will be supportive in helping you access our services, for example providing longer appointment times, or appointments at quieter times of the day.
Please tell us by completing this form.
